Shari Packer, LMSW, C-ASWCM, CDP, is a seasoned care management and social work professional specializing in geriatric care, grief and bereavement, dementia, and caregiver support. She began her career working closely with Holocaust Survivors, assessing individual needs and coordinating care—an experience that shaped her longstanding commitment to serving vulnerable populations with compassion and dignity.
Throughout her career, Shari has held leadership and clinical roles across hospice, senior living, and skilled nursing, supporting older adults and their families through complex medical, emotional, and life transitions. She has extensive experience helping families navigate dementia and cognitive decline, caregiver stress, family dynamics, chronic and acute medical conditions, and transitions of care. Known for her thoughtful guidance, Shari helps families make informed decisions, coordinate services, and access the resources needed to promote safety, independence, and quality of life.
As Director of Aging Services at Intent Clinical, Shari leads concierge care management for older adults and their families. Working alongside Intent Clinical’s multidisciplinary team, she partners with families and their trusted advisors to develop proactive care strategies, coordinate services, and provide personalized support throughout every stage of the aging journey.
